yeah, like bloodnose says... either way you are welding, but my thought would be cut out as far as you need to to get rid of all rust and weld into that... What you see is usually the tip of the iceberg and likely if you have rust 3-5" from the fender lip, you have many serious rust problems and the car is probably not salvageable... at least without a huge welding effort...
Once all is said and done (painted etc), I highly recommend getting the car oil sprayed (and inside every cavity).
I had to give up on our winter beater due to rust. It was oil sprayed every year but the places that didn't get sprayed were the problems (eg floorboards rusted from the inside). I did some hack job repair work that lasted a number of years but if you don't get rid of all the rust, even with POR15, it does come back eventually. You can slow it down, but not stop it. When the drivers side upper control arm broke (rust), and the other side ready to let go also... the amount of work was not worth it given the state of the rest of the car.
Every unsprayed car I have seen up here was rusty everywhere. A bad fender is likely just the tip of the iceberg. I would pull carpets etc and look things over carefully before proceeding.
