I have the exact same problem as you did according to your post below. The bolt towards the firewall was completely pulled out while the other two bolts are still on but mounting holes on the bracket are ripped. Did you replace the bracket? You mentioned that you welded it back but I don't know if it would be strong enough in the long run. And guess what, I have the same set up as yours, rubber mount and polyeurethane mount on engine side. Now it makes me wonder if the problem is inherent in this type of set up. BTW, my car is 1993 Aero.
