from the heated back-plate (Apparently the mirror adhesive like the headliner adhesive, is programmed to immediately start disintigrating after the factory warranty expires).
On my old '86 the glass fell off on the passenger side and on my current '91 the driver side was barely hanging on by a small strip of the original adhesive at the top before I noticed it flapping in the breeze. I re-glued it with thermally conductive silicone adhesive.
Obviously, if this is the case with your mirror, no amount of tightening will help. Try pulling outward on the glass and see if there appears to be any separation between the glass and the back-plate.
