I tightened the nut last week, car was quieter for a day, but then got noisy again so I had to investigate further. Found that one side of the cast iron flange on the aft end of the catalytic converter was cracked (hidden by rust flakes) and would bend when the nut was tightened, so the joint would loosen up again after being tightened.
I found a suitable "split flange" at my local auto parts store, Walker 36130, which is slightly larger than perfect for our car's 2-1/4 ID (2-3/8 OD) exhaust system - the next size down was a bit too small, you would probably need to deform the pipe to put it on. The welds on the existing flange are large enough that this piece will work, and the car is quiet again - we'll see how long this lasts.
Teaching my kid to drive manual transmission probably did not help this piece!
