I've got 327k on my '87 Turbo with the original head gasket. I've been pretty diligent on coolant flushes, never push the engine until it's warmed up, and have never overheated it (though it occasionally got close to the red before I replaced the radiator). I've got broken studs too, but fortunately no meaningful leaks have resulted from it.
Conversely, I got a poorly maintained '86 Turbo that needed a head gasket at 109k. It had some bodgy aftermarket fan thermostat installed which leads me to believe it was probably overheating at some point (sad when it just needed a new temp sensor in the rad).
So I guess it can go either way. The HG is a big job (in my opinion) and yours could certainly last the life of the car. You may want to see if a local Saab indy is willing to just replace the studs at a reasonable price first, but if you can't get the work done inexpensively it could make sense to do the whole job if you're planning to keep the car for some time.
