Assuming the LEDs are not flashing when you press the buttons, batteries are the primary suspect. There are two CR2016 flat lithium batteries inside, reachable by prying open the hatch on back of the fob.
My remote fob had become intermittent even with new batteries, to the point where it almost never worked. I pried the unit apart and reflowed the solder joints where the battery contacts attach to the circuit board. (They're visible as the two largest solder joints towards the center). Now at least for the moment it appears to be working reliably.
