I had very much the same symptoms on my 2003 9-5 Arc just about a year ago.
Air box levers, ACC controls, etc all tested good. Thermostat and general cooling system was functioning properly. Both pipes in/out of the heater core were hot.
In my case it ended up being the heater core. The tubes and channels are so small that when they start to block up, less and less coolant will pass through the area of the core, and cold ambient air will offset the impact of any nominal heat generated.
Replacing the heater core made a tremendous difference in my case
