Yup, your synchro is going.
To avoid, you can short shift, i.e. shift at a lower RPM, but that's no fun. Or, you could learn to double clutch. Get good at it and you won't need them new fangled synchromesh transmissions.
What you're trying to do with double clutching is match the engine and transmissions speeds. "back in the day" tachs had marks showing your shifting speed as well.
While you are upshifting from 2nd-3rd, let out the clutch and blip the throttle...push the clutch back in and it should slide in nice and easy.

That's the cocktail napkin course, I'm sure there is a lot of info on the web and this site that will give you more insight into it...I was forced to learn how by my dad, who thought synchros were for weenies :)
