NOx is caused by high combustion chamber temperatures. At these high temperatures, C02 bonds break to from 0x free radicals which readily bond to the nitrogen in the air, forming NOx.
High combustion chamber temperatures are usually caused by a lean burn condition, a blockage in the exhaust system or, much less commonly, a malfunctioning cooling system.
I am suspicous of a small leak in your induction system, either a leaking induction gasket or a vacuum leak. Common leak points on the 2.3 are the vacuum lines running to the SAI system, the hoses running from the throttle body for the idle air control and the throttle body sealing o-rings. Since you've replaced the vacuum lines, I'd look at the idle air control hoses, PCV hoses (and grommet) and throttle body o-rings. Lastly, I'd verify that your intake manifold gasket is in good condition.
If you absolutely rule out a leak in the induction system, I would look into the exhaust system. A failing catalytic converter or plugged muffler might slow down the exhaust enough to cause higher combustion chamber temps and throw off your NOx.
After that, I'd verify that the cooling system is in excellent condition and that your cooling fan is working.
