You could find a upholsterer and have them take one off and use it as a pattern. The Dmitry Platonoff http://photo.platonoff.com/Auto/ site has a nice little bit on making seat covers.
The actual swap of covers isn't not so easy. There are stiff wires that hook the cover to the foam. I would assume it will fit but after years of wear on the cushions you might have a distorted wear pattern in the cover. I think you might be better off getting the one repaired and putting it back on instead of swapping.
Might even be cheaper to track down a seat that is unworn. New covers are crazy. check e bay for prices.
