In my experience, they will total the car when the cost of repair reaches about 75% of the value of the car.
Sometimes people would rather have the car totaled than undergo major repairs. If you're in that position, and you're close to the 75% mark, you should ask the insurance company to authorize tearing down the car to look for additional damage. There is almost always additional damage in my experience.
If you want them to fix the car, you should get them to start work before they poke around for additional damage. The insurance company will still authorize the expenditure to repair the additional damage, but by that time they'll already have a significant investment in making the repairs.
Of course, if the car is totaled, and you think it might be worth making the repairs, you could arrange to buy it back as Eric suggested. I don't have any experience with that and I don't know exactly how it works.
