Sometimes it's because people are adhering to normal inetrvals when severe is called for (that's 85% of us). But if its free at the 10K, people don't want to pay for every other. Many lease cars in the late 90's had no service included and the lessee never even opened the hood. This doesn't apply to the Corolla case, but it does to a bunch more.
Maybe JiffyLube, et. al. have just put such a cut on the quality of their oil that even 3K won't save you. If the car companies are downsizing components and using the barest minimum quality fluids (remember, the dealer will always let you opt for the better oil if you pay)...why not JiffyDude?
For a one-off case, there's always the chance somebody threw in a bottle of additive the killed it...some poor soul in NG900 did that once. But it is starting to happen so often to so many makes...
But underengineering, and especially underspecifying an interval/quality (no doubt both beancounting decisions to keep 'fee maintenance' cheap) are clearly at play. VW made their pans and filters smaller and for a couple years specified an oil that was nowhere near good enough. PCV has been debated as the problem with Saab and Toyota.
If it is beancounting, I don't think anybody will ever find the 'magic bullet'...it'll be a different specific cause in each case. What hurts is that the manufacturers limit what they'll cover, leaving owners of models with similar engines out in the cold.
