they are too short, going through too many layers of old roof, happened to be placed where there is a gap in the sheathing (can happen on roof decks doen with boards instead of plywood) or there is another problem with the roof. My roof is due for new shingles, but the only popped nailed were where there were 3 layers of shingles from past "repairs."
Leaks are not good. If it is around a fan that is in the roof, then the fan isn't flashed correctly or the flashing and cement around it has failed. If the fan is in your ceiling (I have one like this) there should be no leaks. It means the roof is leaking somewhere. It may just be showing up at the fan, which is why you need to get into the attic and do a visual inspection.
Multiple layers of shingles on a roof can be a problem. Did the roofers remove your old shingles last year? I have a valley that leaked last winter due to snow build up. When I looked at it last summer I found 3 layers of shingles, lots of gaps, prime for leaks from ice dam build up. I pulled it all off, put down ice and water shield and new shingles. No leaks now.
When you go into the attic look *everywhere*. Water travels, sometimes a long way.
