The house we bought 1.25 years ago was previously occupied by heavy smokers. Attempts to wash the walls was futile due to the amount of smoke residue - it was plain disgusting and would have taken weeks. The floors were sanded down and I used Zinnser dewaxed shellac as a sanding sealer before doing water-based poly. For the walls & ceilings we used copious quantities of Zinnser BIN shellac-based primer, followed by zero VOC (i.e. zero odor. wonderful invention) latex paint. Note - if you go really thick with the BIN make sure to get a pole sander and roughen it up with some 120 grit sandpaper. Thick BIN can develop a smooth surface that can cause adhesion issues for the latex. Mind the fumes as the ethanol solvent evaporates...it's especially bad when doing the ceiling.
The result: zero smoke odor, and only 3 or 4 spots where some smoke residue got thru some thin spots of primer and leaked thru the top layer. We wiped those down a few times and they've stopped. For some reason, contractor-type people look at you funny if you mention you used shellac primer everywhere.
As a side effect, shellac is an excellent vapor retarder and last winter we had 40-50% humidity inside with no need for humidifiers.
