I, too, have received the Proof of Claim form for my 2011 9-5 that I purchased in Aug 2011, so the car was covered under warranty when purchased. Similarly, I have paid only for scheduled maintenance and have completed the oil changes myself. Currently, the car has 18,960 miles and in the next month will need another oil change. I have struggled with determining an amount to claim. As you have pointed out, it could be contingent on expenses that have not occurred but could potentially occur in the 36,000 mile/36 month warranty period. I have tried to determine a value for the warranty but there are to many "what ifs". Seems to me there would be an amount a manufacturer builds into the price of the car to cover warranty issues, maybe 1-2%. I don't know. Even if this amount could be determined, what is the likelihood we would even be awarded any amount. However, I am still going to submit a claim. On the form it appeared to me that receipts are to be submitted with the claim, but, I think, as you pointed out a contingent claim can be filed. I did not find the form to be clear as to whether a claim can be submitted without receipts or backup. It has been a week since I read the form, but I don't recall seeing an area for contingent claims. If there is, could you point out where this is located on the form. As far as the amount to claim, I am definitely going to include the scheduled maintenance costs. I think I will research what issues there may be with the Buick Lacrosse and Opel Insignia as a way to determine potential issues and attempt to assign amounts to any potential issue and consider a contingent claim. For the 9-4X, researching issues with the Cadillac SRX may provide items to include for a contingent claim.
