I would rank Refrigeration, Automobile and Hospital Hygiene higher than vaccination in terms of benefit to public health. Refrigeration and Automobile enabled much more dynamic food market, stimulating larger scale division of labor and much higher output, so people can have an abundance of fresh food like we know today. Fresh and nutritious food is the most important factor behind improving public health. Just look at much of Africa's poorest parts today, they are getting the vaccines but still dying of malnourishment due to the lack of electricity (to power the refrigerator) and the lack of transportation to move food to a dynamic market that would stimulate more production and timely delivery.

Hospital hygiene not only dropped surgical mortality rate from 80% a little over a century ago to the lower single digits today, but also saved millions infant and new mothers' lives.

The group immunity dynamics works for stable virii, but for something that mutates as frequently as the flue virus, the preparation of vaccine strain is more of an art than a science . . . and frankly I can't imagine profiteering motives don't come into play.
