Yes, enclosed transport is expensive, even within the US, so you shouldn't count that against the transaction. When shipping my car from California to Chicago, I also received a wide variation in quotes for open transport (car is not pristine). Much of these games are due to some companies owning their own trucks vs. brokers hoping that they may be able to book a low-cost ride on someone's truck.
The PPI is optional, but is probably something that you want to do with any expensive remote car purchase, although $400 sounds pricey.
The other fees are less than $1000. If the car is really a good buy, then this will eventually be insignificant in the overall scheme of things.
