Epo (erythropoeitin) is a polypeptide hormone produced by the kidneys to stimulate red blood cell (RBC) production. Recombinant epo does have some subtle structural differences from natural epo, and there is a test which supposedly distinguishes between these. However, epo has a very short half life, meaning that it disappears rapidly
So the test used for this is the hematocrit, or % of blood volume taken up by RBC's. This value does increase naturally with aerobic exercise, so they look for values which are above what would be expected.
Lance Armstrong actually did use epo, but to stimulate his RBC production after chemotherapy. Recombinant epo was developed for just that sort of thing, as well as for patients with anemia from kidney disease. I doubt he needs to take it still for any medical reason and have no reliable information on whether he takes it now.
