... and there was no way for an astronaut to go out on a spacewalk and inspect the tile for damage. I know this because I asked the question last week at work. This was exclusively a research flight, unequipped to do such a thing.
Also, tile repair on-orbit is not something that can readily be done. If you realize you have tile damage, basically your option is to fly a different re-entry profile in order to reduce risk to the damaged area. Changes to the re-entry profile pose additional risk in themselves. It was my understanding that a judgment was made whereby the risk due to the potential tile damage was determined to be smaller than the risk associated with a re-entry profile change.
The bottom line is that space shuttle flight is risky business. Calculated risk is taken all the time. Unfortunately, sometimes it bites you. More unfortunately, it also often takes a tragedy like this to teach people that safety, and the well being of people, should always be paramount, no matter how compelling the budget or schedule might be. Maybe we should all give some thought to how this applies to each of our everyday lives, in the memory of those who gave their own lives today.
