....It doesn't have to be a poorly machined thread to be an efficient stress concentration. 8) Those look like nice rolled threads to me, nice shiny smooth roots, no abrupt transition into the shank where it transitions into the body. The 45* split direction says it was a tension failure, rather than a torsional one from the nut binding or anything like that.
But yes, overtightening could have played a role.
