Problems when car engine is cold

Hi, My 88 Saab 900 T is having problems when the engine is cold. When ever the engine is below the green area on the temp gauge the car will surge from about 100 rpm to 2500 rpm, and keeps stalling. Once the car warms up it runs fine (occasionally it will surge 50 rpm up or down but not often). So every time I leave the car for 2.5 hours I find my self holding the accelator down keeping the rpms at 2500 till it is warmed up which is very annoying. Now this is what I have done, I replaced the AMM with one from a car I know is fine, that did not work. And today I tried cleaning my AIC and when I found that didn't work I got a new AIC and tried that. That also did not work. Now what should I try? One thing I noticed is that the Crankcase vent. hose is melted and there is a hole in it, would this cause the problem? Also does anybody know where i can get a GOOD diagram for the vacuum hose routing?
