Sometimes (on both Saab's and Volvo's) it is a little plastic rivet that is inserted through the webbing of the belt that break off.
That will allow the belt to retract too far into the reel and lock up.
Is it any way that you can pull the belt out in any position so that you can study the webbing for evidence of a hole through the belt where the rivet has been.
It is also a plastic guide inserted into the seat backrest that the belt runs through. That guide is used as a stop where the rivet stops against. If that guide is missing, the rivet may pass by too far in.
