Any scientist worth his/her salt will tell you that you need large sample sizes to draw any conclusions regarding any test. The IIHS takes a sample size of one (one car, one crash test) and then extrapolates from that one test that all identical cars made from that manufacturer will behave similarly. One would like to believe that this is true and that their tests are reproducible, but even under the "controlled" conditions under which the IIHS conducts their 40mph offest crash tests, there must be some variables that are not accounted for.
In fact, Volvo railed against the IIHS when they tested an S80 (I think)that had some problem with the airbag or something and flagged the car for that problem. They retested and the problem didn't recur (was an isolated incident). Just goes to show you how accurate the IIHs can be.
That-and the fact that the IIHS is sponsored solely by the insurance companies, who are more concerned about trying to reduce claim costs than promoting real safety.
