1. Worn out? Probably not. But at 90k EVERYTHING is showing some wear (rack, tie rod ends, suspension bushings, ball joints....). Extra toe does put pressure on all the parts to cut down on slack.
2. 0.18 (degrees) per wheel is the max - spec in the old WIS is .14 deg +-.04. So it is right at the edge of factory spec. So yes, tire life will be reduced compared to having less toe, but you are getting more stability in return. Total spec is 2.6mm which really is hardly any toe at all, so I don't think it will hurt much.
Steering wheel alignment is always a sore point. DO NOT let them adjust it by attempting to move the steering wheel on the shaft - it may not even be possible, and it is definitely incorrect. Steering wheel alignment (centering) is done by adjusting each tie rod end, the same amount in the same direction, to bring the wheel back to center without affecting toe.
