How many miles on the vehicle? Belt? Tensioner?
I have seen many 6+ OG9-3s and 2 short belt OG9-5s (4 cylinders) have this type of failure. Usually the belt stretches to the belt tensioner outer limit and/or the spring within the tensioner is fatigued allowing the belt to briefly slacken on startup. For some odd reason, it tends to fall off the water pump pulley and lodge between the wp pulley and timing cover.
Always replace your drive belt, tensioner assembly and both idler pulleys every 60k with OEM parts. On the factory short belt 9-5s, the mount for the lower idler pulley is there on the timing cover. I dont even stock a short belt for a 9-5. Every single one gets a long belt setup.
