A “monitor” is one of the predefined tests executed by the car’s OBDII system, which if able to run to completion, and out of specification outputs detected from sensors, can result in generation of a code and possibly turn on the check engine light. The number and type of monitors varies by car but generally there are more on newer models. Three monitors run continuously: comprehensive component, misfire detection, and fuel system. Others such O2 and CAT run only once per trip after the enabling criteria has been met.
As I understand it OBDII is primarily focused on assessing and reporting the status of the car’s emission system, though it is possible to infer causes for drivability problems. Put another way, a $100 code scan tool is not a replacement for a $3000+ Tech 2.
