I'll take a shot. It might be the balance shaft chain/tensioner, or it could be the timing chain. The timing chain tensioner could fail, but they are pretty simple devices. I would pull the valve cover and inspect the chain and sprockets for wear. Make sure the chain guides are intact (not broken) and the sprockets don't have any unusual wear patterns, like a hook. Remove the tensioner and check for the extension service limit (14mm). Don't forget to remove the tension on the spring before removing the timing chain tensioner. It will be difficult to check the balance shaft chain, but try to look down the timing cover to see if anything is amiss. Change the guide on the valve cover before you put it back on if you can't find anything else wrong.
