Your alternator should be putting out closer 14v. The regulator is built into the unit and the brushes are accessable by removing the regulator. The brushes are probably worn out or below 5mm or no longer have sufficient spring tension pressing against the armeture to make proper contact. You could remove the regulator and check the brushes for wear and replace, but they have to be soldered. I had the same problem recently and determined it would be better to replace the entire alternator. I also recommend checking both the alt.arm bushings and the alt. mounting bushings for wear. If worn they allow the alternator to vibrate, they're cheap and well worth it for a quality repair.
