Could it possibly be a motor mount causing both problems? I had inner driver shake and replaced the front motor mount first and it curred most of the problems. There was some residual damage but its mostly cured.
Just thinking if there was enough play to pop the gear shift out of third. Either way... check the front mount or all three and see if they are in good shape.
