Actually, the appraiser is the one that looks at the car and makes the determination of how much to deduct from the actual cash value (it's called unrelated prior damage, or UPD for short). Sometimes, appraisers won't even go that far to total a car.....they'll look at the damage and the overall condition of the car and go from there. Sometimes, you'll get an appraiser that's just hellbent on totaling a car, and he or she will go out of their way to find every little ding, dent, nick and scratch to put it under. Usually, though, the appraisers are so busy, they won't bother to look under the car, unless the car got smacked hard in the rear, and the appraiser's looking at the rear body and floor and/or exhaust. But if you got T-boned in an intersection, there's a pretty good chance, unless it was really obvious, that the appraiser would never see the rot in the floors.
If the rust is seen, the appraiser calculates the cost to repair the rust and adds it to the total UPD. That figure is then deducted from the actual cash value. So, it's not really a reduction by percentage, it's a reduction by dollar value based on the cost to repair the bad stuff.
Depending upon the rules in your state, even if your car is declared a total loss, you can opt to keep the car and repair it, or keep the car and part it out, if the damage is really bad.
