I think a lot of the issues people have with these transmissions is due to too infrequent atf changes. Now I do a drain and refill with ford type f every time I do an oil change. I have similar symptoms as you do and I've noticed some improvement with more fluid changes... that said, there is an now an optimum temperature range where it works well. Too cold (first start up of the day in winter) and it's clunky/slow to shift. Too warm (driving for half an hour or more) and the shift point goes back up to about 45mph. In between it shifts fine at 30mph.
On top of that there is kickdown, so with that connected, the actual shift point will vary depending on the throttle... but my numbers above are the minimum shift point, and I assume that is what you are saying also. It is very aggravating when the damn thing won't shift until 45, and I bet driving around in second all the time makes things worse...
yes, change the fluid. ford type f only! turbocon86 outlines a more thorough method to do the change on the CE registry site. I just do a triple change (drain/refill/drive repeat 3x) and then change it regularly.
