If the spacer in question is the one I'm pointing at in the photo then that usually means that the mainshaft nut on the front end of the mainshaft has come loose. It can also be caused by the stack shims having broken up and left the building. Both of those things happen in these units but usually with higher mileage or as a result of a bad rebuild. It is not generally caused by sloppy pinion bearings because the pinion assembly nut would have to move closer to the pinion gear and even with very sloppy pinion bearings that won't happen (although there's a first time for everything). You can tighten the nut but doing it in the car would be a huge, frustrating effort and if the stack shims are gone (and there's no way to determine that) you'll just create another, worse problem. The sensible fix is a rebuild and if done promptly, before other damage results (which it will) the cost would be low and the resulting unit would be a nice one. Please don't drive it until it pops, these units and the parts to rebuild them are getting scarcer every day.
