Have you had the flywheel resurfaced? maybe more than once? curious if you are at or beyond the spec for thickness.
this moves the pressure plate further from the slave mount and it can make the slave hyper extend to do it's work, and that can push it out past the O rings in the slave and kill them. If you have another slave installed I would at that time get the pressure plate measure too.
