The condition of the belt was fine at both failure events. In addition the bearings of the various pulleys involved also were in acceptable condition. The first tensioner failure was due to failure of the tensioner mounting plate, and I have read that this part tends to fail. The second failure was due to one of the bolts that fasten the mounting plate to the engine block failing and breaking in two. Upon inspection of the failed bolt my mechanic stated that there was evidence of rust on part of the bolt, indicating inproper metallurgy in the manufacturing process of the bolt.
