Fuel run-out puzzle

This all started after an indy replaced my fuel pump:
I have been living with a fuel run-out issue for years. I want to fix this now after running out of gas with my wife last weekend.
Here's the thing! I have to fill my tank at the 1/4 tank mark or the engine will run out of gas. I know the capacity of the tank is 17 gallons and the most I can put in is usually 11 gallons !!!
The pump hash mark lines up with the trunks, so allignment seems right. Also, this is reducing my driving range to 230 miles or so.
Besides the pump siphon not being on the lowest tank point, is there anything else that can cause this issue?
I was thinking a pressure issue if the return line check valve is bad or the FPR diaphram or the regulator to the charcoal canistor is bad??
If any of you veterans can solve my puzzle - I will be very happy!
I have never pulled the pump out to check it. Dash guage seems accurate as to level and little reserve light comes on at start-up.
