Turns out the plastic pipe that connects one of the heater hoses near the false firewall was cracked. In fact it broke apart as soon as a little finger pressure was put on it. I'm surprised there was nothing visible with the engine running - as soon as the pressure tester was pumped up coolant started dribbling out back there.
A little bit of a pain to get to, but much better news than water pump, heater core, or head gasket. Those plastic pipes really seem like a weak point. Might be better long term to use a short length of copper pipe, but for now I had spares from Eeuroparts handy so just put in another plastic tube.
