Yeah, it's not the coupler, though I have seen them fail. The heater core is leaking at the plastic end tank. I might pry the tabs back and remove the end tank to see what happened. Oddly, this one has a brass fitting between the two heater hoses. I must have done that when I did the head gasket a couple years ago. I think I only had one new coupler and I didn't want to take a chance on an old used one.
