Often a 'hot' cam does not have larger lobes. The size and shape of the lobes define 'lift' and 'duration'. Hot cams usually change the timing of when the valves open and how long they stay open. These changes can change power, where the power peaks, or doesn't peak in the power band (RPM). Changes in the cam timing can greatly effect fuel economy and emissions, and how the engine runs and responds to throttle input at different speeds. The most obvious example is the classic hot pushrod V8 with an idle so rough it barely runs, lousy throttle response off idle, but huge horsepower in the 3-4000 RPM range. Stock engines usually are developed for smooth idle and response and broad power band for most street uses, and in the last 30 years to meet increasingly stringent emmissions requirements.
