The main disadvantage is cost. It's not cheap to build a 120 HP motor.
The only risks I can think of with the motor itself, are if you try to overbore too much- then you run into head gasket problems amoung other things. Or if you try to push the RPM too much past 7000.
Smoothness- the two cam grinds most readily available make it a little lumpy at idle, but not bad.
Loudness- a Y type, extractor exhaust is louder than stock. You can quiet it to your liking with the muffler of choice.
Gas mileage- depends on your right foot! It seems there is a curve- a mild HP increase with the right intake/exhaust can increase it. Past that, and it will decrease. If you up the compression too much, you're into high test gas, which is pretty pricey.
In sum, the little V-4 has a very constricted intake/exhaust system. Alot of the performance boost is making the engine breath better. Bigger valves, porting the heads, bigger exhaust, and a decent carb. Adding the cam, balancing the rotating elements. All these are positives and don't really affect reliability.
The other item to consider is the transmission. It tends to be the weak element in the equation and will need some attention.
I have a similar engine in one of my Sonetts, built by Jack Lawrence of MSS. No regrets. I love it.
