The ECU cannot adapt the mixture enough, withing mapping norms, to get the oxygen sensor happy. So it has an adaption fault and states too lean.
Ever clean your injectors? Could be part of the problem. Use a 20 ounce bottle of Techron Concentrate. Repeat 2x per year. This will cost $10US. Most auto retail shops have this, Walmart has the smaller bottles too. Make sure it is the 'Concentrate'!
The fuel pressure could be low. Replacing the fuel filter at 100K is normal preventive maintence. However, these usually are good for well over 100K. But fuels can vary from place to place.
Oxygen sensors can be fooled if washed on the exterior with exhaust gases from a leak. And these get their signal grounds via the exhaust pipes.
Sometimes the sensor needs to be backed out and then reseated to get a good contact. An ohm meter can be used to determine if there is a ground problem, and to locate the point of resistance, such as the down pipe to turbo joint etc. Once detected, the problem is easy to solve, if there is such a ground problem.
O2 sensors normal last well beyond 100K.
