Maybe the freewheel lever (nylon crescent-shaped one on top of the transaxle) just isn't moving far enough to engage the locking sleeve all the way. I've heard of people solving that one by taking off the lever and elongating the slot so it can move a bit farther. Then you clamp it down with a bolt and hope it stays put.
A more robust barnyard-engineering solution is to find or make a metal plug like the one pictured (at least I HOPE the picture works; I borrowed this photo from JLW's "Saabstories" website, by the way.) As you can see, this is a machined plug with an off-center hole in it, and a tab to bolt it down. The idea was that you'd fit this in place of the nylon lever, turn it until the locking sleeve was REALLY seated, then bolt it in place. Not sure how you'd make one without a lathe, but if you're a machinist (or have a pal who is) this might be an easy project.
