Check Engine Light and Codes

Went to Autozone today to have the codes pulled on my 1999 9-5 sedan, and they found the following three codes:
P0138 - O2 Sensor Circuit High Voltage (Bank 1 Sensor 2)
P0140 - O2 Sensor Circuit No Activity Detected (Bank 1 Sensor 2)
P0442 - Evaporative Emission Control System Leak Detected (small leak)
It sounds like the oxygen sensor is failing, but what is the correct part # for the replacement part, and how difficult is it for a new DIY'er to replace? Any special tools needed?
