Tensioner went at 48,000 miles

Mine just went at 48,000 miles. The belt and tensioner were changed by the dealer at 60,000. Now with 108,000 the tensioner went. Luckily, the engine was OK. The car was running fine and then it started to run rough at idle with a P0300 code (random misfiring). About 10 minutes later I started hearing a "flapping" sound from the engine and took the car to my local indy. He showed me the tensioner that failed.
