I just went through this in April. I replaced the pulleys myself, reinstalled everything, then it popped off again! I took it apart a second time and looked closely at the mechanism - it appears that one of the arms that the pulley goes on had gotten bent slightly, which was making it pull off the pulley when near the top of it's movement. Bending it carefully I was able to make the mechanism work better.
While I was raising/lowering to test it, I also noticed that it closed much more tightly against the top of the door than the other windows on my car. I made it close less by loosening a 10mm nut that looked like an adjuster, moving it in tiny increments until the window seemed to close more normally. I'm not sure that this is what that nut is for; it seemed rather sensitive, and I can't describe a good way to adjust it. I just moved it, tried it, moved it, tried it until everything seemed to work OK. No problems with the window since then.
