It's usually not possible to pull the radio and cage as a unit without damaging the dash fascia. When you remove the cage normally, you are compressing it into the space the radio occupies.
With the tools, there's sometimes a trick to using them. Insert the tools until they click in, then push the ends of the tools sticking out away from each other (spreading them...), and then the radio should be released. You may need someone to pull via the cassette slot as well.
