Rear brake pads – odd wear pattern

My rear brake pads have worn in a strange pattern. The outer pad has two areas near the outside, top and bottom, that are more recessed. The inner pad has the same pattern, but those areas are larger. The rotor has a matching pattern. I started with fresh Akebono Euro pads and new rotors. Later on, the guide pins were modified to remove the rubber and add a metal sheath. For now, I just swapped the pads. Any idea why this happened? Bad rotors or pads? Rotors were Saab I think. All was installed by a reputable Saab indy.
Am I losing stopping power? Should I swap the rotors too to be sure before the new pads get ruined? Are the cheap Wearevers decent or should I get better?
I also noticed the outer pad wore much quicker than the inner one. I presume this means something is not sliding properly so the outer pad is dragging or perhaps the inner one is not biting. The flanges on which the pad edges sit are somewhat worn so as not to be fully flat, especially for the inner pad. I don’t understand how that happened. The inner pad does not contact the top of the floating caliper. Is that is how it is designed or is something not right with the Akebono fitment?
