Transmission problem???

Hi all,
Car is a 2003 9-5 Aero 5-speed. I had the transmission replaced already, I think it was around Fall of 2010. my mechanic at the time told me it looked like the leaking oil had eaten away the rubber bushings/engine mounts so the transmission was just sitting on the frame and eventually failed. Well I'm at about 156,000 miles now, my wife was driving it to work today and coming off the highway, when she shifted from 5th she said it "popped out" and then she couldn't get it into another gear. Finally she did but she fought with it to get it to work (finally). I picked her up and attempted to drive it home. Couldn't even get it out of the parking lot. I had it in reverse and when I stepped on the gas, it was almost like I had the emergency brake on or I was stuck in mud. You could feel the car surging and the RPM's rising, but it wouldn't go anywhere, then suddenly it would "break loose" and go back about a foot. I had a hard time getting it into other gears, but finally did, circled the lot, and put it into another space.
Does it sound like my tranny is toast (again), or is it maybe something fixed by replacing the engine mounts? And if it is toast, what do you do with a 156k 9 year old car with a busted tranny???? I just moved from the northeast to the gulf south so it's not like this is a saab hotbed. Thanks,
