My wife and son were out practicing driving in our 1999 9-5 4 cylinder last year and he may have popped the clutch too hard or other jerky move and the belt slipped off. As a shoulderless pulley on the water pump, I think it is a crappy design! I had replaced the leaking water pump 4 years ago with one from NAPA and every replacement belt since then seems to drift over slightly to the engine block and shaves off about one belt rib before settling down fine. No further cutting into face of timing belt cover which can happen with bad results (oil leak!) I hear. Maybe its just a defective one from NAPA, but a better design from Saab or aftermarket source should have had a better pulley with shoulders on both sides to keep the belt better captured from drifting in towards the engine or off the other side! The power steering pulley has shoulders as well as the crankshaft and A/C compressor, but maybe I'm missing some mechanical concept that all the idler, tensioner and water pump pulleys running off the smooth backside of the belt have to be shoulderless.
