If you monitor RockAuto and/or this site, you will find tensioner assemblies available for little more than the cost of the pulley. At that point, why wouldn't you replace it? The tensioner pulley is actually the most heavily stressed of the idle pulleys - it has the smallest included angle (for the greatest load) and the smallest diameter (for the highest revolution count).
I too regrease the used pulleys, but I still install a new grooved pulley, a very inexpensive part and more heavily loaded. I did swap in a regreased 2nd hand smooth pulley based on the same logic: It is the lease heavily loaded of the three.
