Intermitent operation like that usually points to the fan. When the brushes become worn out, they don't make good contact with the commutator and the motor stops moving. Bumps in the road would occasionally shake the fan enough to get the motor moving. You can check for worn brushes by pulling out the fan and looking at their length. If they are very short, then they are probably worn out. It is possible to replace the brushes, but when it happened to me, the commutator was so worn, that the new brushes I put in would stick out too much in the holder and get cocked and the brushes could not move freely. A new fan solved the problem.
